To HSOP Class of 2012:

On behalf of the Harrison School of Pharmacy (HSOP) I extend our warmest welcome to you as student pharmacists in the Class of 2012. We are extremely enthused about having each of you join us. The purpose of these web pages is to inform you about orientation, policies and procedures, and documentations and forms you will need to complete prior to the start of Fall Semester 2008.

CPR, First Aid, Health Records Report, and PPE Profiles
Incoming student pharmacists are to provide these completed forms/documentations to Ms. April Staton. Please note that the HSOPs Health Records Report on this website is different from the Auburn University Medical Information Form.

Students who have not been previously enrolled at Auburn University are also required to complete the Auburn University Medical Information Form and send it to the Auburn University Medical Clinic, Medical Records Department. Ms. Smith has previously sent you the Auburn University Form, either by email or in the regular US Mail.

Documentation of Medical Insurance: In order to protect the health and financial security of student pharmacists, and to facilitate their academic success, the Harrison School of Pharmacy (HSOP) has adopted a policy that requires all student pharmacists to have health insurance (Please see HSOP's Policy located on this website). It is important that you read this Policy carefully so that you will clearly understand its terms and conditions.

Please note that you must provide documentation of medical insurance to Ms. April Staton. Documentation may be faxed to 334-844-4346 or emailed to statoag@auburn.edu. If you have insurance coverage under a parent's or spouse's policy, the documentation provided must clearly identify that you are one of the individuals who is insured under the policy. If you do not have a policy or insurance card that identifies you as an insured individual, you should be able to obtain such documentation upon request from your parent's or spouse's insurance company.

Student pharmacists may purchase insurance through the Auburn University Student Health and Accident Insurance Plan, and those student pharmacists who do not have other coverage must purchase at least the basic health plan. Information on this plan is available on line at www.studentresources.com. This website will allow students to purchase online and immediately print a receipt documenting their insurance purchase. The Office of Financial Aid has increased annual cost of attendance for pharmacy school; this allows additional loan money for purchase of medical insurance for those students that might need it.
