Briefly describe innovation

Steve Bandy owns an independent pharmacy in Illinois. He has incorporated three sit-down counseling areas and one stand-up counseling area. He has used these areas for blood pressure monitoring services. He is currently implementing a software package called CareStream Patient Management services as monitoring parameters for blood pressure, peak flow readings, significant interventions, and screening information. In these counseling areas, the pharmacist counsels and monitors the blood pressure readings. There are currently 20 - 25 patients utilizing his services permanently. He plans to implement a Diabetes Care center and soon to be incorporating an Asthmatic counseling area.

Reimbursement rate : $50 per year ; $3 per visit

Is documentation required? Yes. They document all visits by written records on each profile.

Specialized training required? Not any for the blood pressure counseling services besides a Pharmacy degree. The Diabetes Care center counselors has to certified through a continuing education program by NARD.

Software/instrumentation required? Yes. He is currently implementing a monitoring package by CareStream. Also, (3) manual blood pressure devices are needed.
