Briefly describe innovation

Background information: Community store.

Innovations:
1. Hypertension Monitoring- uses specialized software to monitor blood pressure. Readings are saved in the software system until patient has a physician's appointment, then a detailed report is generated and sent for physician review.
2. Diabetes Management- teaches patients to use glucose monitors. After one week, patients are called to check for problems or questions. All glucose meter readings are saved and then downloaded into main computer, allowing for adherence screening, physician updates, etc. Twice yearly, monitors are cleaned and inspected, and patients are "reminded" how to use their meters.
3. Refill Reminders- sends patients more than five days late for refills of chronic medications a reminder via mail. This list of patients is generated by the pharmacy computer system.
4. Home Visits- visits patients in their home if requested. Brown bag reviews, counseling sessions, etc. are conducted at patient's request. Due to this service, Mr. Bartone has many elderly patients.
5. Asthma Management- newest interest. Mr. Bartone plans to teach patients about their asthma, how to monitor asthma using peak flow meters, and how to use maintenance medications.

Payor(s): Patients, Patients' Relatives, Third Parties.

Reimbursement rate : Two programs available: $25 per visit or $20 per month in a yearly contract. No extra charge for home visits.
