Briefly describe innovation:
Cognicare Pharmaceutical Care Practice
Their primary goal is to develop individual patient care and to have a way to track and
monitor patient charts to go beyond the prescription filling site. They dispense some
drugs, but they want to change the "count and pour"system even though it still
predominates. They strive to do more patient interaction and forum to improve patient
interviews. They desire to implement patient care. Their future goal is to team with
physicians to share patients who need added attention to better their care.

Mostly no, but in Washington they have the "Care Project" in cooperation with
the University of Washington and DSHS, the state medicaid program. This program is not
permanent but it is a pilot program to determine how pharmacist intervention has worked
and if it really improves patient care. Pharmacists can bill the state for cognitive
services. Examples include pre-filling insulin syringes for patients with vision problems
or monitor blood glucose levels to people who are incapable of doing it themselves.
Reimbursement rate
Usually about $4.00 if under 10 minutes and $6.00 for anything over 10 minutes.
Is documentation required?
A basic form is required for the "Care Project" program. The prescription for
these services have certain NDC numbers that go along with the drugs so that the state can
keep tract online.
