Mr. David Blair has designed an innovative pharmacy. His idea focuses on the patient after they leave the physician's office. He started this program because after a patient leaves the physician's office he/she does not know if the patient filled the prescription, if the patient was counseled, or if the desired effect of the medication was achieved. Therefore Mr. Blair decided to start this program that will help to manage the outcome of the patient. The program is piloted by a doctor's office. After the patient leaves the office they come to Mr. Blair's pharmacy to fill their prescription. Five-ten days after the prescription is filled Mr. Blair calls the patient to follow-up with them about how the prescription worked. He then notifies the physician of any complications that the patient experienced. This program will help monitor patient compliance and help with outcome management of patients.

Reimbursement rate: $3.00-$5.00/call; fees are negotiable

Documentation is done by fax. Doctor documents patient diagnosis and therapy and faxes it to the pharmacist. The Pharmacist documents patient outcome and faxes it back to the doctor.
