Briefly describe innovation:

Mr. Cziraky works with a company called Health Core. This company deals with managed care activities, which operates by programs to manage outpatient care. Health Core also participates with clinical drug research of investigational drugs (Phase 2 and 3 Clinical Trials). Health Core is a group that works closely with the physicians, and patients of this facility are only accepted on a physician referral basis. This company is not a clinic. Some of the areas that the practice deals with are as follows: CV risk management, hyperlipidemia, smoking cessation, diabetes management, and hypertension. With hypertension management, Health Core provides blood pressure monitors, and monitoring services. They also make changes in patient medication regimens, with the changes being approved by the physician. They focus on the patient, physician, and selves triangle of care, meaning that they use all of these sources of information to provide maximal patient care. They also provide services for rheumatology, pulmonary, and ophthalmology. The Health Core staff is comprised of the following professionals: 2 residents, 3 pharmacists, 1 pharmacist working on a Pharm. D. degree, 2 nurses(1 LPN, 1 RPN), 1 Data Management Specialist, 1 Pharmacoeconomic Specialist, and 6-7 physicians. They see on average about twenty to thirty patients per week

Method of Reimbursement:

Payor(s): Health Core does not presently charge patients for the services they receive.

Reimbursement rate: The money to cover the patients care comes from research(on a per capita basis), but Health Core is currently working to get third party payment.

Is documentation required? Yes, they record notes of intervention in the patients' charts. The

physician will then sign off the pharmacists intervention recommendations. Only the physician guides the changes. The interventions are also recorded in a database.

Specialized training required? Mr. Cziraky holds a Pharm. D. degree, and completed a 1-year residency with a managed care focus.

Software/instrumentation required: Health Core uses Microsoft Access, and other data bases
