Briefly describe innovation

Previously, Mr. Drystad and twenty other care sites started a pharmaceutical care program three years ago. Only ten sites remained involved at the end of the term. A contract was formed with Blue Cross/Blue Shield to provide coverage for this care. The two main care programs built involved calcium supplements and bronchial inhalers. However, in addition to the previous, he is now involved in a "Modified Pharmaceutical Care Prescription Review." Involvement from Health Partner's, an HMO in the surrounding area, is being sought. Their is a meeting every two months to discuss the issues that would make this effort efficient. Convinced, the HMO has agreed to a contract. Mr. Drystad and three other pharmacists will be caring for one hundred and twenty patients. The breakdowns of the clients include the following: thirty with COPD, thirty with asthma, thirty with hypertension, and thirty with heart disease. Four other pharmacies are interested in getting involved in the treatment plan. However, they will only be trained for one day, while Mr. Drystad and his co-workers were trained for three years. He states, "At least it is a start to improving care." In addition, he is making many other additions to his pharmacy. He has installed a lipid monitoring program that involves taking blood samples to decide HDL & LDL, while checking glucose levels in the process. He also has a focus help group dealing with diet and exercise. Merck is also providing a machine that can check the bone density in patients. He is contemplating hiring registered nurses' to help with these new procedures.

Method of Reimbursement: Currently, he is not getting paid for providing care under his latest care program. However, he is working on a yearly capitation plan that would provide so much/patient/year.

Is documentation required? No. However, serious drug related problems will require documentation in the near future.

Specialized training required? No.
Mr. Drystad states that the pharmacists' who have done the most for pharmaceutical care formatting only have a B.S.

Software/instrumentation requires? No
However, a testing of software is being done in his pharmacy at the present time. It is still under modification before going to market. It is fairly inexpensive with a price of around $195.
