Tracking Pharmacist Interventions. Mr. Dudding has developed a system within the Lakeland Regional Healthsystems Hospitals that keeps track of interventions made by clinical pharmacists. The network of three hospitals and an extended care facility currently uses a mainframe program called HBOC to document specific cases in which their pharmacists have had a significant impact. HBOC is a quick, user-friendly documentation module that is "on-line" and immediately integrates transcribed information between all of Lakeland Regional's facilities. They previously used a relational database called PARADOX (which Mr. Dudding helped develop) for tracking Rph interventions. This system had the advantage of producing better quality reports, complete with presentable graphs, data tables, etc. It required, however, more complicated entering of information and was not as immediate as the HBOC system. Documenting significant interventions made by clinical pharmacists helps evaluate specific cases and situations in which the pharmacists were involved, and more importantly serves as a means of proving the indispensability of pharmacists in quality health care.
