Briefly describe innovation:
Our practice site is located in Nashville, NC, the county seat of Nash County, and a town of approximately 4000 people. We are located 10 miles west of Rocky Mount, NC, population approximately 50,000. Our area is primarily rural and many of our patients have no insurance coverage. Our pharmacy is of a traditional nature in a 5600 square foot building. In addition to pharmacy services we offer a full service florist, a gift shoppe, an express business center, and jewelry and cosmetics. We are located beside the Nashville Medical Clinic, presently in an expansion mode from 2 doctors to 4.
Our pharmacy was reengineered in January 1995 upon aligning ourselves with the Pharmacare (now JAS encounters) organization, adopting their physical layout. We had our pharmacy counter cut down to counter height and built a 9 X 9 patient counseling "bullpen" directly connected in front of the pharmacy counter equipped with 2 stand up and 2 sit down semi-private counseling areas. Our technicians handle all patient information gathering, keying and filling of prescriptions, calling physicians offices for refills, and many other tasks delegated to them. Pharmacists function strictly in a patient counseling, disease state management role.
Our Diabetic Shoppe, in existence since November 1992, is a mainstay of our pharmacy practice. Another full time technician manages and runs this entire operation, including all meter training, imparting of limited nutritional information, performing blood glucose testing, ordering of all supplies and merchandise, and handling a mail service component that has grown tremendously over the past year. We presently have 2,500 persons with diabetes on our quarterly newsletter mailing list and sell over 30,000 blood glucose test strips per month. We work closely with many educators in our state, providing them with monitors so that they can train patients "hands-on", then ordering their supplies form our shoppe. We are the premier diabetes specialists in eastern North Carolina.
Our immediate plans are to employ two more pharmacists in July 1997, one of which will be solely responsible for implementing revenue streams outside (or indirectly) of our normal dispensing process. We plan to implement asthma education in our school system, smoking cessation clinics, hypertension clinics, and any other disease states we feel appropriate to our setting.

Payor: Patients, but in the future we will be implementing billing to third parties.
