Briefly describe innovation
Hospital pharmacy. The pharmacy has eight pharmacists that spend half their day dispensing and the other half counseling. They offer counseling in anti-coagulation, asthma, lipid therapy, refill clinic, monitoring drug therapy, adjusting dosage, and follow up care. Patients are referred to the pharmacists by physicians. The pharmacists have a contract with the physicians and they split the fee.

Reimbursement rate: Payment is negotiated with the insurance companies.
Is documentation required? Yes, a record is kept on patients medical chart. Also progress reports are kept on the charts.

Is specialized training required? Yes, all pharmacists should have their PharmD and have completed a residency program.
