Care Point member. Ms. Jagodinski currently provides three different Pharmaceutical care programs. The level one program is the basic total Pharmaceutical care package. It contains self care consultations, disease management, adherence programs, and detailed counseling with each new prescription. Level two is a program for patients that don't currently get their prescriptions filled at her pharmacy but want one of her care programs. Level three is the most comprehensive level. It essentially is the level one care for the elder on the verge of having to be admitted to a nursing home. The adherence part of level three is the main difference from the level one care plan. It provides, for example, weekly bubble packs for these type patients. Ms. Jagodinski also provides an asthma management program. She offers the asthma program at the store or will go to the of a disable patient. Ms. Jagodinski uses Cognicare software provided by Care Point. This software is used to document all of her interventions and maintains total patient profiles on all of her patients.
