BRIEFLY DESCRIBE INNOVATION:
Denny Jorgensen has set up a pharmaceutical care program in his pharmacy. This
pharmaceutical care program allows the pharmacist to become more interactive with the
patient. Denny's staff includes two pharmacist and four technicians. They have changed the
workflow by empowering the technicians with more responsibility. The technicians handle
the full dispensing process. The drug labels generated by the technician are sent directly
to the counseling area. Therefore, the pharmacist does not deal with the medication at all
other than to check for accuracy. The pharmacist functions face to face with the patients
providing these pharmaceutical care services. Denny says that the patients are responding
well to his new program. Denny's program is patient oriented because the pharmacist seeks
out the information from the patient and addresses the patients problem. Occasionally, the
patients sign a release form to allow the pharmacist to obtain the patients medical
records from the doctors. However, the pharmacist usually just calls the doctor and
discusses the patient's therapy. Denny hopes to improve his pharmaceutical care innovation
and eventually obtain reimbursement from either the patient or the third parties.
METHOD OF REIMBURSEMENT:
PAYOR: Denny currently receives payment for the drugs dispensed.
Denny is currently working on a third party project with 27 other Iowa pharmacies. This
project deals with Blue Cross Blue Shield. They are enrolling samples of high risk
patients in their pharmaceutical care programs. The high risk samples involve patients
with asthma, diabetes, hypertension, and angina. The pharmacist is documenting all
interactions and outcomes of these patients. They hope to be reimbursed by documenting the
impact of pharmaceutical care on the outcome of these high risk patients.

IS DOCUMENTATION REQUIRED?
Documentation is required. Denny originally started documenting on paper. Now, he is using
a computer system to do the documentation. Denny uses this system to keep track of the
progress of his high risk patients involved in the Blue Cross Blue Shield study, and other
patients.

SOFTWARE/INSTRUMENTATION REQUIRED?
Although software is not required for this documentation, Panora Pharmacy is currently
using a computer system called Data Information System that comes from Pharmacy Care
Outcomes in Minnesota. This system can use the patient's medical records and document
patient progression and outcomes for future use.
