Briefly describe innovation

Family Pharmacare, an independent pharmacy, offers disease state management programs for asthma and other conditions. This article will focus on the Asthma Integrated Management program, or AIM. The program originated in 1993. The pharmacist meets with the patient monthly for six months in the acute disease state management phase and follow-up care is provided in the next management phase as needed. In these visits, the pharmacist answers questions the patient may have, reviews the medication profile for appropriateness of the medicines, and teaches the patient and/or the guardian the correct procedures for maximizing therapeutic outcomes. The main goal of this program is to help people take control, or manage their asthma. This program currently has several patients, primarily children and young adults. This program is definitely successful in expanding the pharmacist's role in patient care.
