Briefly describe innovation:
Mr. Panar is a Triple Board Certified Pharmacist. That is, he has been a registered
pharmacist in the state of New Jersey for twenty years, a clinical consultant for nursing
home for twelve years, and a certified Diabetic Educator for four years. He practices
pharmacy at Lynwood Pharmacy and works approximately seven hours a day. He also owns Life
and Health Education Center and sees patients by appointment usually about two to three
days a week. The pharmacy is 800 square feet and fills about 100 prescriptions daily.
Other than prescriptions, the pharmacy only sells professional products such as vitamins,
dermatologic products, and a new line of herbal therapy (Mr. Panar is educated in
homeopathic remedies and can offer advice to patients. Mr. Panar started Diabetic Training
in 1980 when a physician asked him to monitor his wife that was not controlling her blood
glucose. At the Education Center, Mr. Panar has a private office for counseling and a
group area that will accommodate twelve patients for group presentations. Mr. Panar offers
consultations in diabetes, asthma and hypertension control and does blood glucose
screening, cholesterol screening, and will soon offer Coumadin Filtration. Additionally,
Mr. Panar does therapeutic intervention and makes recommendations to physicians on drug
therapy and untreated disease management.
Method of Reimbursement:
Payor(s): Currently, Mr. Panar bill third parties for approximately 40% of the Diabetic
Consulting, and the remainder is cash fee for service from the patient. Mr. Panar also
receives cash fee for service for asthma and improving patient outcomes consultations.

Documentation Required:
Documentation includes basic review charts, patient history, and pharmacist's comments for
all intervention and contacts to physicians. At this time, all of this information is kept
manually.
Specialized Training:
In order for Mr. Panar to receive third party reimbursement, he is a Certified Diabetic
Educator. In addition, he has been a consultant and representative for Squibb, Novo, and
several other companies.
Software/instrumentation Required:
The Education Center offers On-line Computer Software and In-house Loading that Mr. Panar
uses for group and private diabetic presentations.
