Briefly describe innovation: This program was designed as a pilot between two health care organizations, Memorial Medical Hospital and E2M-Health Service. This program contained 200 subjects that had Diabetes Mellitus. The program consisted of four three hour seminars that were taught by doctors, pharmacists and nutritionalists. Following these seminars there was a meeting period of the subjects so that they could be evaluated on their medical history, exercise , stress, and diet history. They also had a sensitive foot test and a laboratory examination of their Hemoglobin A1C value. From this information the evaluators put the subjects into three groups, High, Moderate and Low Risk Patients. Based on this categorization the subjects were then monitored and counseled by the staff as to what they could do to change their lives for the better of their health. This pilot was a success and Mr. Perez hopes that it will be implemented to certain H.M.O.s in the community.

Reimbursement rate Initial Fee of $225.00 then $5.85 per year under the pilot program. *Note: once this program spreads to the community it will be more expensive and most likely be charged to the individual or the individual's third party.*
