Briefly describe innovation

Care Stream Encounter Program. In this pharmacy, individualized patient care is provided. The pharmacy has undergone extensive remodeling with the addition of one booth and a second private room. The approximate cost of the renovations was $10,000. Fourteen patients have currently enrolled in the Care Stream Encounter Progam. These patients get their blood pressure, weight, and medication monitored per visit. Technicians do the entry work so that the pharmacist is free to work one on one with the patient. All medications are reviewed by the pharmacist, who passes the prescription on to the patient. The pharmacists are members of the PCCAA and do a significant volume of comounding. An additional pharmacist will soon be added , as well as a cholesterol screening program. The Care Stream Encounter Program is being promoted to Insurance companies on a Risk -Share plan. The Risk- Share plan promises to reimburse insurance companies if the patient is hospitalized within one year while under the Care Stream Encounter Program.
