Diabetic Care Center: services provided include, 1) monitor training ($35/30 minute session) 2) follow sessions to evaluate technique 3) physician referral when therapy problems exist 4) Educating the patient on: nutrition, medication, insulin inject ion, eye care and yearly exam, foot skin care, management of complications.

Asthma Care Center: service provided include: 1) correct use of inhalers 2) compliance monitoring 3) help on modifying lifestyle 4) follow-up to check for problems 5) regular check of therapy for referral back to the physician if therapy fails 6) t raining on use of peak flow meter.
