Briefly describe innovation

Mr. Gary Reynolds concentrates in four different areas in his innovative pharmacy. He counsels patients in the area of diabetes where he teaches type I diabetics how to inject insulin as well as balance exercise and diet. With the type II diabetics, he counsels on the proper uses of each drug for that patient. With all of his diabetic patients, he teaches them how to use a blood glucose machine and the how to manage their blood sugars. Another area that Mr. Reynolds works is asmtha. He teaches all of his patients the importance of asthma management, drug management, exercise, and monitoring of their condition. In the area of hypertension, he teaches his patients how to monitor their blood pressure as well as give drug consultations with them. Mr. Reynolds also performs cholesterol monitoring in his pharmacy.

Is documentation required? Yes Every one to two months Mr. Reynolds sends an evaluation form via his fax machine to the patient's doctor. The form contains the report period, patient name and data, and monitoring parameters. These parameters are preventative screening, compliance, interactions, and care plan status. He also marks one of two columns beside these parameters in either the out of range or normal areas. If Mr. Reynolds would like for the doctor to reply back, he leaves room at the bottom of the form along with what he would like for the doctor to reply to.
