Briefly describe innovation
Focus on Disease State Management of high risk patients.
Michael Soiland is the operational pharmacist for MedOutcomes. MedOutcomes currently
focuses on four disease states: Diabetes, Hypertension, Asthma, and Hyperlipidemia. The
company plan is to add two new disease states each year, and plans to add Estrogen Therapy
Replacement and Depression next year. Mr. Soiland sees patients in a retail setting by
appointment, in a room adjacent to the pharmacy. During the appointment, he assesses the
patient's compliance by reviewing the applicable refill history records, he reviews side
effects with patient, and he interviews the patient on the signs and symptoms of the
particular disease. He also takes pertinent physical measurements of the patient, such as
blood pressure readings, cholesterol levels, blood glucose or hemoglobin AC levels, and
peak flow meter readings. He also records measurements like pulse and weight.
Method of Reimbursement
Payor(s): mostly the patients-- MedOutcomes is working with BCBS of Richmond, but their system is implemented in other states where reimbursement is from third parties as well as employer groups.
Reimbursement rate: Set fees for each service (fees vary for different services)
Is documentation required? Yes. The software records the relevant info.
Specialized training required? Yes. Home study which takes about 24 hours of reading time. Then, there is on site training for the different disease states. There is physical assessment training, guidelines training (which involves writing letters to physicians), setting up appointments guidelines, and communication training.
Software/instrumentation required? Yes. MedOutcomes has developed software for pharmacists to implement pharmaceutical care in their setting. This software has a text generator that contains guidelines for the pharmacist to ask their patients. By having all the information, the pharmacist can inform the patient's physician of any positive or negative progress prior to the patient's next visit with the physician. This helps to facilitate communications and lets the physician know that the pharmacist is supporting them in the role of patient care.
