Briefly describe innovation:
Claxton Pharmacy is a small independent pharmacy set up as a professional pharmacy as opposed to the traditional retail setting. Claxton Pharmacy has an office that services approximately 1000 patients. The pharmacist is currently working on different projects which include a model for reimbursement for pharmaceutical care. The pharmacy contains three separate areas for counseling with a sit down desk for interviews and an office for more private interviews. Computer software is being utilized for dispensing with technicians entering the medications into the computer in order to free up time for counseling.

Method of Reimbursement:
Payor(s): Claxton is currently working with other insurers in the state to develop better methods of reimbursement for cognitive services.

Reimbursement rate: The cost of pharmaceutical care is currently being absorbed in the cost of the prescription.

Is documentation required? Yes. Pharmacist uses patient care notes as a method of documentation.

Specialized training required? Yes. ICPC-Pilot Study. Staff underwent 35-40 hours of training involving documentation and communication . The pharmacist had well over 80 hours of training in these areas.

Software/instrumentation required? Yes. The pharmacist used both paper documentation and computerized charts. Data Matter and Health Outcomes are two programs utilized for these processes. Insurance programs in conjunction with the University of Minnesota are also being utilized.
