Briefly describe innovation

Redesign pharmacy to contain two consultation booths and a video patient education center. On each prescription the following services are provided: DRR, DUR, drug-drug interaction, drug monograph, patient consultation by pharmacist on medication use, s ide effects and how to minimize them, necessary lifestyle changes. When necessary, patient outcomes are monitored and documented, i.e., blood pressure, blood sugar levels, peak flow meter and blood cholesterol. Interactions are documented using the S.O. A.P. method.
