How to become an AUSP Patient

We have a New Patient Form that all new patients will need to complete. This is not the same form used by the Medical Clinic. We do not share patient information between the Medical Clinic and AU Student Pharmacy.

  • Bring in, email, or fax the completed form and a copy of the insurance card to the AU Student Pharmacy at 334-844-4969.
  • We accept cash, check, credit cards, FLEX spending cards, and we can bill your AU BURSAR account. We cannot extend personal credit to any patient.
  • Manufacturer coupons will be honored ONLY if they are submitted at the same time as the prescription so that they can be processed together.

How to send your prescription to the pharmacy

  • We can fill prescriptions from any doctor or prescriber (in-state or out-of-state) as long as they are licensed in the United States, not just AU Medical Clinic physicians.
  • New prescriptions can be sent by your physician’s office to the pharmacy via fax, telephone, or e-prescribed. We also accept prescriptions from patients who bring them in to the pharmacy, as well as those sent via regular mail, and/or campus mail. If utilizing a mail service be aware that prescriptions may be occasionally delayed and/or lost. The pharmacy does not accept responsibility for prescriptions lost in the mail, and in this event, we would need a new prescription from your doctor.
  • Refills: Please call the pharmacy and use the automated system or speak to one of our friendly staff.
